Ralph Ebbon Williams, 51, of Daniels Farm Road, Trumbull, is charged with driving without a license, evading responsibility and improper use of a license plate.
Shelton Police said Williams drove off Mohegan Road and hit a tree on May 14, and walked away.
A witness later discovered the accident and Williams was arrested, police said.
Williams' bond was set at $500 for an appearance in Derby Superior Court.
