HIA along with the Health Department Release Community Health Assessment and Improvement Plan
The 2025 Community Health Assessment and Improvement Plan for the Greater Bridgeport Region is Available

Trumbull, CT (December 5, 2025) – The Greater Bridgeport Health Improvement Alliance (HIA), a collaborative of hospitals, community health centers, health departments and other organizations in the area, has released their most recent Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). This alliance consists of partners from Bridgeport, Easton, Fairfield, Milford, Monroe, Stratford and Trumbull. The plan reflects more than 18 months of collaboration, engagement, and shared commitment with community partners.
The 2025 CHA integrates a variety of data sources, including community surveys, focus groups, and interviews, and focuses on local and regional resources, as well as barriers that prevent residents from achieving their fullest health potential. Through a data review and prioritization process, three key areas emerged: Mental Well-Being, Preventive Care and Quality of Life, and Strengthening Communities.
The 2025-2028 Community Health Improvement Plan (CHIP) builds directly on the identified priorities and outlines goals, actions, and metrics designed to foster meaningful, measurable progress for each of the focus areas. The CHIP reflects a commitment to removing barriers, addressing root causes, and creating lasting improvements in the conditions that shape Trumbull’s health. - Coordinate and participate in health fairs and other community events
- Provide evidence-based training courses (e.g., mental health first aid, QPR and others)
- Provide wellness workshops, training and prevention screenings (e.g. Matter of Balance)
- Advocate for programs and policies that improve and support preventive care and quality of life.
