Lassiter Students Need to File Medication Forms

Lassiter High School students with medications to be taken at school for Asthma, Diabetes, Seizures, Allergies/Epi-pens, and other chronic illnesses, need to file forms annually.  

New forms will be needed for the new school year. You can obtain a form at  the CCSD website: http://www.cobbk12.org/centraloffice/studentsupport/schoolhealth/  

Lassiter School Nurse Audrey Dinoff is available to assist you or answer questions. Contact her if your child has an issue that she needs to know about at school. She can assist and advocate for your child. 

Dinoff will be available to meet with parents and accept medications in the clinic on Friday.

Many supplies are used by students visiting the clinic. Please consider donating any of the following: 3 oz. disposable cups, boxes of tissues, elbow/knee bandages, peppermints, ginger ale.
