The McHenry County Department of Health (MCDH) is investigating any illnesses that occurred on or after Feb. 7  and have an association with Crystal Lake Montessori School, 3013 S. Country Club Rd. in Woodstock, or at any events at an alternative location sponsored by the school.

The investigation is ongoing and therefore information around the total number of cases of illness and the cause of illness is currently not available.

MCDH is requesting that everyone who ate food either at a Crystal Lake Montessori School classroom event or from a school-sponsored event, regardless if they developed symptoms, complete a confidential survey available at redcap.link/foodillnesssurvey. MCDH is also asking that anyone who is associated with the school and became ill but did not attend any of the school-sponsored events on or after Feb. 7 also complete this survey as it will aid in the overall investigation.

Parents who complete the survey on behalf of their child, please answer the questions respective to their experience. Information collected in the survey is confidential. In addition, the survey database used to collect this information is HIPAA compliant. The survey will only take a few minutes to complete and is important to help understand what happened to cause illnesses.
