Five Lake View Restaurants Fail Food Inspections

Patch combed through food inspection reports of local restaurants, cafes and grocery stores. The results are available here.

Patch examined some of this October's food inspection reports from Lake View, and found that eight restaurants in the neighborhood have failed inspections since Oct. 18.

The information has been collected from publicly available data published by the Chicago Department of Public Health's Food Protection Program. The data is available here

To fail an inspection, the establishment had to have serious violations that weren't correctable during the inspection. A failed rating does not mean that the restaurant's license is automatically suspended. 

Flub a Dub Chub's, 3021 N. Broadway

Comments: No evidence of rodent or insect outer openings protected/rodent proofed; observed live fruit flies in the dishwash area and the bar areas — approximately 40 live fruit flies were observed on the walls, ceiling and equipment in these areas; found approximately 20 rodent mice droppings in cluttered furnace room; front double door windows from the dining room is not insect/rodent proof; General detail cleaning needed.

Johnny O'Hagan's, 3374 N. Clark

Comments: No evidence of rodent or insect outer openings protected/rodent proofed; observed approximately 25 live fruit flies in the kitchen area and the second floor food prep area, dry storage room; Several exit doors to the establishment are not insect/rodent proofed; sneezeguard/barrier must be provided for the open-front smoker equipment and display for shellfish. Please note that Old Crow did pass the follow-up inspection.
