Restaurant Inspections: Boat Motor at Deli, Grocery Store Sushi
Another round of restaurant inspections are showing a few Lake View favorites completely missing the mark.

Patch reviewed heath inspection reports in Lake View, and a new slew of businesses were examined since our last inspection report.
Information was compiled using publicly available data published to the website of the Chicago Department of Public Health's Food Protection Program.
Restaurants can pass, pass with conditions or fail using a standardized procedure. Establishments that pass don't have any critical or serious violations. Those that receive 'pass with conditions' had serious violations, but the violations were corrected during the inspection. Failed establishments had serious violations that weren't correctable during the inspection. A fail rating does not mean the business' license is automatically suspended.

Newport Bar & Grille

- A disorganized basement caught this health inspector’s eye, along with a few other messy details. Employees at the restaurant were asked to repair refrigerators in the kitchen, clean the floor under the cooking equipment and replace broken wall tiles under the dartboards.
Fitness Formula Clubs (FFC)
3232 N. Halsted St.
Inspection Date: Nov. 30
Pass with Conditions
- A serious citation was issued to the fitness club when health inspectors couldn’t find a certified food manager or a health certificate. The club was instructed to enroll in the program, considering it sells fruit smoothies and shakes. Inspectors also asked employees to remove the clutter from the food storage area so rodents wouldn’t create a home there.
New York Deli
2921 N. Clark St.

Fail
- Health inspectors paid a visit to New York Deli and found a huge number of violations leading to the restaurant's failure. Some of the more notable comments included a serious citation for failing to fix a sink issue from a 2010 inspection, clean dishes being stored in the same location as dirty dishes and—for some reason—a boat motor was found in the restaurant’s basement. Combined with leaky pipes and employees missing their hair nets, New York Deli has some cleaning up to do.
Candyality
3425 N. Southport Ave.

Pass
- Outside of tidying up a bit, this Southport candy store passed the inspection without a hitch. According to the report, employees were asked to clean the floors in the store, as well as declutter a back hallway.
Avenue Tavern
2916 North Broadway
Inspection Date: Nov. 30
Fail
- This popular bar and restaurant in East Lake View received a serious citation because it didn't replace the loose floor tiles in the service area noted in an earlier inspection, according to the report. Combined with a dirty cooler, a floor covered with “dirt and debris” and an “extremely slow drain” in the women’s bathroom, the health inspector failed the eatery.
Caribou Coffee
3300 North Broadway

Reinspection: Pass
- Health inspectors went back to Caribou on North Broadway after finding about 40 insects behind the food-prep area, as reported in early January. Both the North Broadway and North Halsted locations have failed health inspections recently.
Jewel Osco Sushi
2940 N. Ashland Ave.
Inspection Date: Dec. 5
Pass
- Leave all stigmas about grocery store sushi at the door because the sushi counter at the Jewel on Ashland is in fine shape. The only comments the health inspector had for employees were to place a “raw and undercooked” food advisory in public view, repair a cooler door and clean the floor.

- Looking for a sweet treat? Because this donut and ice cream combo is a clean place to munch. According to the report, a health inspector asked employees to make sure cleaning cloths are stored in sanitizing solution and to buy a thermometer for the dairy cooler.
