Custom Home Care, a family-owned and nurse-managed home care agency serving Chicago’s seniors for more than three decades, has been named a winner of the 2025 Chicago Star Small Business Award, presented by the Office of the City Treasurer, Melissa Conyears-Ervin.

The Chicago Star Award celebrates small businesses that exemplify excellence, resilience and community commitment. Winners were selected from citywide nominations recognizing local enterprises that strengthen Chicago’s neighborhoods through innovation, job creation and positive social impact.

Founded 31 years ago by Tamara Jackson, RN, in the basement of her Chicago home, Custom Home Care has grown into one of the city’s most trusted and highest-rated providers of senior care. As a licensed Home Services Agency and Home Nursing Agency, the company has served over 10,000 clients, helping seniors age safely and comfortably in their own homes. Today, Custom Home Care employs more than 300 team members, many of whom are immigrants, creating meaningful work opportunities and elevating the caregiving profession.
