O’Shaughnessy’s Passes Health Inspection; Urban Interests Fails

Patch examined inspection reports in Lincoln Square over the month of October. Information has been compiled using publicly available data published to the website of the Chicago Department of Public Health's Food Protection Program.
Restaurants can pass, pass with conditions or fail using a standardized procedure. Establishments that pass don't have any critical or serious vocations. Those that receive 'pass with conditions' had serious violations, but the violations were corrected during the inspection. Failed establishments had serious violations that weren't correctable during the inspection. A fail rating does not mean the business's license is automatically suspended.
All inspections are done on a routine canvass unless otherwise noted.

Polygon Café, 5204 North Clark Street
Inspection date: Oct. 30
Result: Pass with Conditions
Comments: The reach-in freezer was not working in the rear preparation area. The manager emptied the freezer and will repair it. The basement walk-in cooler floors were in poor repair; floors must be smooth and easy to clean. The walls in the sushi prep area need to be repaired and more lights need to be installed in the basement.

O’Shaughnessy’s Public House, 4557 North Ravenswood
Inspection date: Oct. 29
Result: Pass
Comments: This visit was the result of a complaint. There weren’t any ice bin covers at the bar and there was dust build-up on the preparation area reach-in coolers. The ceiling light covers also need to be cleaned.
Aroy Thai Restaurant, 4654 North Damen
Inspection date: Oct. 29
Result: Pass
Comments: This visit was a follow-up from a past inspection where the restaurant did not pass.
Dalia Food Market, Inc., 4661 North Broadway
Inspection date: Oct. 29
Result: Pass
Comments: This visit was the result of a complaint. The inspector observed dirt and dust build-up on the floor, under and around all shelves and along the corners and walls throughout the restaurant. A thermometer was missing from the reach-in cooler and the basement was cluttered. All items need to be at least 6 inches off the ground for storage.
Lindo Guadalajara Restaurant, 4761 North Clark Street
Inspection date: Oct. 24
Result: Pass
Comments: Management needs to replace the rubber door gaskets in the prep coolers and repair holes in the basement floor and walls. There was also rust build-up on the mop sink water handles.
Ravenswood Community Daycare, Inc., 4908 North Damen
Inspection date: Oct. 23
Result: Pass
Comments: The caulking around the washbowl sink needs to be repaired and the vent cover in the adult washroom needs to be cleaned.
Mobil, 4638 North Damen
Inspection date: Oct 23
Result: Pass
Comments: Management must defrost the ice cream freezer where heavy ice has accumulated, remove the soda crate being used as a pallet and provide a regular pallet with space under it to clean the floor. A visible thermometer needs to be displayed inside the ice cream freezer.
Urban Interests, 2124 West Lawrence Avenue
Inspection date: Oct. 19
Result: Fail
Comments: Coolers did not maintain a temperature of 40 degrees or below and there was wastewater back up at the compartment sink. The inspector found a gap more than .25 of an inch on the bottom of the door—it needs to be sealed to prevent insects and rodents from entering. Management must seal the raw wood under the front counter of the service station, provide a lid for the ice bin and install a splashguard between the three-compartment sink. All floors, cooking equipment, coolers, deep fryer, dishes, utensils and sinks need to be cleaned. Gaps between the walls and ceiling around the pipe in the preparation area need to be sealed and the cracked walls in the basement need to be repaired. Thermometers are needed for all freezers and coolers and ‘No Smoking’ signs need to be posted in on the entrance front and exit door.
