Purdue University Northwest Is Conducting A COVID-19 Vaccine Survey

PNW will use the information to provide care and guidance for the campus community.

The Safe Return to Campus Task Force needs your help. As we plan for the safety of the university community, we’d like to know how many members of the PNW community have received a COVID vaccine.

All students, faculty and staff affiliated with the Purdue University Northwest campuses are encouraged to participate in a new verification program once they have been fully vaccinated for COVID-19 to assist in PNW’s efforts in managing the health and safety of the campus community.

This is a new survey! Even if you filled out the spring survey, we would like you to complete this survey.

Take the Vaccination Survey

Three COVID-19 vaccines are now available in the United States that have been deemed safe and highly effective by the FDA. All PNW campus community members are strongly encouraged to receive a vaccination as soon as they are eligible to do so in their home state or location. In Indiana, people age 12 and up are eligible under Indiana State Department of Health guidelines to receive a vaccine at any vaccine distribution site in Indiana.

Through a Qualtrics survey those who have fully completed their COVID-19 vaccination — two shots of Moderna/Pfizer or the single dosage of Johnson & Johnson – can share that information confidentially with the Safe Return to Campus Task Force. You will not be asked for identifying information such as your name or PUID.

PNW will use this information to provide care and guidance for the campus community, specifically related to ongoing testing, quarantine and isolation, for those fully vaccinated individuals who have been exposed to the coronavirus. Thanks in advance for helping us collect this information.
