Southside Woman Forged ADD Prescription: Police

Woman faces a felony charge after she allegedly altered a doctor's prescription form to obtain a bottle of Adderall one month early, according to an arrest record.

A South Bethlehem woman is accused of altering a prescription to obtain a drug used to treat Attention Deficit Disorder from a city pharmacy one month before she was authorized to, court papers say.

Lauren Worozilcak, 27, of 2171 Conolly Ave., altered the β€œfill after” date on her doctor’s prescription form from April 22 to March 22, according to a staff member at the doctor’s office where the prescription was written, Bethlehem police Officer David Bowerman alleges in an affidavit of probable cause.

The fraudulent prescription for Adderallβ€”a stimulant composed of four amphetamine salts used to treat ADDβ€”was then filled at the CVS at 327 Woodlawn Ave., the affidavit says.
