- Chipotle Mexican Grill, 5 National Ave., Malvern: 3 violations on Oct. 17; sanitizer dispensers not refilled, soda machine not clean, and Dumpster dirty.
- Homewood Suites, 12 E. Swedesford Road: 7 violations on Oct. 17: employee beverages in the refrigerator, cheese sticks not refrigerated, ice machine with residue, ice build up in freezer, no certified food manager.

- Malvern Treatment Center, 940 King St., 9 violations on Oct. 19: nonfood surfaces not clean, gap between floor and wall, walk-in cooler shelf broker, chlorine sanitizer residue on dishwasher, using single-use gloves without wash, broken door, ceiling tiles stained.
