The Austin/Travis County Community Health Plan has been a leading effort in improving the health and wellbeing of Austin/Travis County residents since 2012 and we’re currently completing a county-wide assessment.

Join our Virtual Community Discussion on Thursday, Nov. 18th, 6:30 - 8 p.m., via Zoom, or tune in and call in to our special radio show hosted by KAZI FM 88.7 on Friday, Nov. 19th from 6 - 7 p.m.

The Austin/Travis County Community Health Plan is a collaborative planning initiative implemented by organizations throughout the Austin/Travis County area. Organizing partners include Austin Public Health, Travis County Health and Human Services, Central Health, St. David’s Foundation, Ascension Seton, Baylor Scott & White Health, Integral Care, The University of Texas Health Science Center at Houston School of Public Health in Austin, Austin Transportation Department, Capital Metropolitan Transportation Authority, and The University of Texas at Austin Dell Medical School.
The third iteration of our Community Health Assessment began June of 2021 and will conclude in March of 2022. Austin Public Health will share results with community residents, partners, and stakeholders in the Summer and Fall of 2022.
