Up To 23 Violations, Mold Found: Manassas Restaurant Inspections
Health inspectors found foods that may not be from an approved source, mold in the ice machine and more violations during August.
MANASSAS, VA — Some restaurants in the Manassas area recorded violations during recent health department inspections between Aug. 12 and 21.
Health inspectors conduct most restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.
For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

Here are the most recent inspections from the Manassas area, according to the Prince William Health District. There were as many as 23 violations, each at Lapaz African Cuisine and Pizza Boli's.
Health inspectors noted uncertainty about some foods being USDA approved at Lapaz African Cuisine. Shakin Crab had 18 violations, including mold in the ice machine. Another mold violation was found in the ceiling and vents at the Splash Down Water Park Paradise Pizza.

Brooklyn Pizza and Italian Grill: 9987 Sowder Village Sq Manassas, VA 20109

Violations:
- Employees and conditional employees are not trained on reporting requirements concerning information about their health and activities as they relate to diseases that are transmissible through food.
- This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
- Observed a personal care item (hairbrush) stored on a table behind the bar. The person in charge removed the hairbrush from the table and stored it in a proper location.
- The sanitizer concentration in the mechanical dishwashing machine was 0ppm, lower than the minimum concentration needed for the machine to properly sanitize equipment and utensils as specified on the data plate (50ppm).
Lapaz African Cuisine: 8041 Centreville Rd. Manassas, VA 20111

- There was an employee observed washing hands at the prep sink.
- The hand sink in the kitchen area and the hand sink in the dish washing area were blocked by dishes.
- The leaves used for the Waakye rice could not be verified from an approved source. Also, there was invoice from meat distributor for Tilapia, turkey tails and croaker but could not verify if the food was USDA approved.
- An employee was touching ready to eat food items with a food thermometer that was not properly washed, rinsed and sanitized.
- In the walk in cooler and throughout the facility there was food/ingredients that were not covered to prevent contamination.
- In the walk in cooler there was raw food items stored on shelves above and next to ready to eat food items.
- The 3 compartment sink did not have stoppers to properly set up the sink to wash, rinse and sanitize dishes. There was an employee washing dishes in the prep sink without properly sanitizing dishes after cleaning.
- The bone saw was not clean to the sight or touch. Throughout the facility several food contact surfaces that were not clean to the sight or touch.
- There was containers of fish, and eggs that were left on the counter at room temperature.
- On the metal cooling rack there were several containers of chemicals that were stored next to or above food ingredients and or food utensils.
- In the walk in cooler there was bottle of liquid medication stored on the top shelf above food that will be served to customers.
- The facility was not taking the proper precautions to cool food items.
- In the 3 compartment sink there was raw goat meat inside the basin without running water to help thaw. Also on the chairs in the kitchen there was containers of frozen fish thawing a room temperature.
- There was several bulk containers and smaller containers of food items that did not have the name of the items inside and were not easily identifiable.
- In the walk in cooler there were containers of food that were on the floor.
- There were food items, ingredients in nonfood grade containers in throughout the facility.
- None of the employees in the facility had hair restraints on while engaging in food prep/cooking.
- Single service food containers were being reused for food storage.
- There was wood utensils being used as food contact surfaces.
- In the facility there was small stand mixer, 2 door sandwich prep cooler, and 2 door prep table cooler that were not properly working or being used.
- The facility did not have any chemical food contact sanitizer onsite.
- The bathroom in the kitchen was not self closing.
- The overall facility had a buildup of debris, grease covered walls, dust collection on the floors

- The 3 compartments sink faucet is not in good repairs. Observed unapproved and soiled plastic extension attached to the faucet to provide necessary length is attached.
- Facility is storing unnecessary items (unused cold holding unit)to the operation or maintenance of the establishment.
- Some floor tiles in the food preparation and warewashing area are broken not in good repair.

- Observed walk in cooler door is not self-closing nor latching.
- Observed reach in upright freezer with frost buildup.
- Observed leaking faucet at the 3-compartment sink.
- A supply of toilet tissue is not available at each toilet.
- Physical structure is not maintained in good repair. Missing ceiling tiles in multiple locations. Evidence of roof leaking occurring.
- Observed multiple extension cords being used for cold holding equipment.
- Observed that items that are unnecessary to the operation or maintenance of the establishment are being stored on the premises.
- Wet mop is not placed in a position that allows it to air-dry without soiling walls, equipment, or supplies.
- Light bulb in walk in cooler not shielded, coated, or otherwise shatter resistant.

- Intake and exhaust air ducts are not being cleaned at a frequency necessary to prevent contamination by dust, dirt, and other materials in the kitchen above the 3 compartments sink.
- Less than 50 foot candles of light was noted in the dishwasher machine area (15 f/c measured).
