10.              Plan Commission Meeting held June 12, 2012 (Kastner)

Item #5  Approve a PUD amendment to extend the Lease Termination Date for Budget Cinema, 4475 S. 108^th Street for two years to July 1, 2014, subject to Plan Commission and staff comments.  Tax Key #609-9994-013. 

Item #6  Approve a Special Use for the purpose of opening an animal care/pet supply retail shop in the tenant space at 11015 W. Layton Avenue, Suite A, subject to Plan Commission and staff comments which include to consider this a minor change not needing a public hearing.  Tax Key #612-8992-003.

14. Application for a Temporary Class “B” Retailer’s License received from Greenfield Partners of Parks & Recreation to sell fermented malt beverages at the Greenfield Fourth of July Celebration on Wednesday, July 4, 2012 from 11:00 a.m. to 11:00 p.m. at 5151 West Layton Avenue

15. Application for a Parade License, and request to waive the license fee, received from the Partners of Greenfield Parks & Recreation to conduct a Parade on Wednesday, July 4, 2012 with the parade route as follows: Assemble at 11:00 a.m. on South 68^th Street north of West Layton Avenue and south of West Layton Avenue to West Edgerton Avenue with the parade commencing at 12:15 p.m. at South 68^th Street & West Layton Avenue, proceeding east on West Layton Avenue to Konkel Park, 5151 West Layton Avenue, entering the parking lot via the east entrance; eastbound lanes of West Layton Avenue will be occupied by parade participants

16. Application for a Carnival License, and request to waive the license fee, received from the Partners of Greenfield Parks & Recreation to hold the Greenfield Fourth of July Celebration with food/toy vendors, carnival games, inflatables, musical acts and entertainment at Konkel Park, 5151 West Layton Avenue, on Wednesday, July 4, 2012 from 11:00 a.m. to 9:30 p.m. 

17. Application for a Temporary “Class B” Retailer’s License received from St. John the Evangelist Festival to sell fermented malt beverages and wine at a festival to be held Friday, July 27, 2012 from 5:00 p.m. to 11:00 p.m.; Saturday, July 28, 2012 from 3:00 p.m. to 11:00 p.m.; and Sunday, July 29, 2012 from 12:00 noon to 9:00 p.m. at 8500 West Cold Spring Road

18. Application for a Temporary “Class B” Retailer’s License received from St. John the Baptist Armenian Church to sell fermented malt beverages and wine at Armenian Fest to be held Sunday, July 29, 2012 from 10:00 a.m. to 7:00 p.m. at 7825 West Layton Avenue

19. Longshots Sports Bar Inc., Donald D. Habermehl Jr., Agt., requests an extension of their 2012-2013 Combination Class “B” Beer & Liquor License and their 2012-2013 Entertainment License, to the parking lot at 5045 West Loomis Road on Saturday, July 28, 2012 from 11:00 a.m. to 7:00 p.m. to have games as a benefit for Breast Cancer (Longshots Sports Bar)
