All AUSD students need to be vaccinated against COVID-19, or tested weekly. The deadlines to provide documentation or testing consent are January 3, 2022 for students 12 years and older, and January 26, 2022 for students from 5 to 11 years old. In the absence of vaccination documentation or testing consent, students will be enrolled automatically in Independent Study. This form details ways to submit documentation of your student's vaccination, or to begin the consent process for weekly COVID-19 testing.
