Free T-Dap Shots Next Week at Dixon High, CA Jacobs Middle School

T-DAP shots will be available at on Monday from 12:30 – 5 p.m.

On Tuesday T- DAP shots will be available at from 9 a.m. -2:30 p.m.

On Tuesday T- DAP shots may be available at Dixon High School from 11 a.m. – 5 p.m., depending on Nurse Staffing Levels

 A few notes regarding T-Dap immunizations:

• No prior shot record is needed but would be helpful if families have any questions
• Insurance documentation or policy numbers are not needed
• * Students need either a parent on site or a signed note of permission from their parent
• Staff will check to make sure all information is correctly recorded and staff will give students/parents proof of T-DAP immunization to bring to school
• Required for secondary students (Grades 7-12) this school year
