
Ozzy Osbourne, the heavy metal icon, died of a heart attack, according to a Tuesday report from the New York Times.
Osbourne died July 22 at 76, just weeks after his farewell show. His family announced his death, but did not provide details at the time.
A death certificate filed at a registry in London lists cardiac arrest and coronary artery disease among the causes and notes that he had Parkinson’s disease, the Times reported.

Osbourne died of “(a) Out of hospital cardiac arrest (b) Acute myocardial infarction (c) Coronary artery disease and Parkinson’s disease with autonomic dysfunction (Joint Causes)," the certificate reads.
His occupation is recorded as “Songwriter, Performer and Rock Legend.”

The original lineup of Osbourne’s band Black Sabbath performed for the first time in 20 years in July in what the rocker vowed would be his last-ever live performance, five years after he revealed he had Parkinson’s disease after suffering a fall.
