Cal/OSHA Investigating Fatal Petaluma Silo Collapse
Authorities say the worker who died was welding atop the silo.

PETALUMA, CA — Authorities released the name of a man who died Friday after a metal conveyor system he was dismantling collapsed on him. Adolfo Sanchez, 31, of Fairfield, died at Santa Rosa Memorial Hospital, according to Frank Polizzi, a spokesman for the California Division of Occupational Safety and Health, or Cal/OSHA.
Sanchez, an employee of Lind Marine, a regional dredging company, was welding on top of a silo near the D Street Bridge in Petaluma when the conveyor system collapsed and trapped him, Polizzi said.
Bystanders said they saw a large metal tower collapse and heard Sanchez screaming for help, Petaluma Fire Department Battalion Chief Chad Costa said.

Firefighters responded around noon and used a 35-foot ladder to reach Sanchez. Crews lifted the structure to free Sanchez and he was airlifted to the hospital, Costa said.
Polizzi said the investigation into possible safety and training violations would take about six months.
