Eric Javier Ayora, 20, of Center Street, is charged with evading responsibility, failure to drive right and reckless driving.
Police said Ayora was arrested April 13 at Coram Avenue and Center Street.
His bond was set at $500 for an appearance in Derby Superior Court.
