Mohamad Nabile Rahim, 22, of Holbrook Place, Ansonia, is charged with third-degree forgery and fifth-degree larceny.
Police said Rahim took the check in early December and cashed it for $450. He allegedly admitted to the theft and turned himself in to police.
He is free on a written promise to appear in Derby Superior Court.
