Richard W. Sasko, 46, of Perry Avenue, is charged with five counts of failure to change his address.
According to the state's sex offender registry, Sasko was convicted in 1988 of second-degree sexual assault and first-degree unlawful restraint.
His bond was set at $5,000 for an appearance in Derby Superior Court.
