A Derby woman allegedly fled the scene of a two-car crash Sept. 28.
Melissa Kollman, 26, of Paugassett Road, is charged with evading responsibility, failure to drive right, driving without insurance and driving without a license.
Police said the crash occurred at 299 River Road.
Kollman's bonds totalled $500 for an appearance in Derby Superior Court.
