- Observed faucet of three sink compartment in disrepair. CA: Equipment and utensils shall be designed and constructed to be durable and to retain their characteristic qualities under normal use conditions. Replace faucet.

- Observed raw shrimp stored below raw chicken in main kitchen walk-in cooler (improper vertical storage). CA: Raw poultry must always be stored at the lowest level of your walk-in unit as it has the highest required final cook temperature. Item was corrected onsite.
- Observed multiple food items failing to maintain temperatures of 41 degrees F or lower (see "temp. observations"). CA: Items were discarded onsite. Advised that units failing to maintain adequate temperatures be repaired.

CA: Food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from other raw meat and ready to eat food using the proper separation method of chicken at the bottom then beef then pork then seafood and then ready to eat food.
- Observed uncovered food items in RIC/WIC. CA: Cover all foods by storing the food in packages, covered containers, or wrappings.
- Observed prepared food not marked with date of preparation or date for disposal. CA: Food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below.
- Observed build up of food debris on equipment (non-food contact) throughout main kitchen. Must be cleaned to sight and touch.

- Observed chemical sanitizer reading at 0 ppm from the chemical dish washer. Per data plate, the chemical sanitizer must read at 50 ppm to properly sanitize. A chlorine solution shall have a minimum temperature based on the concentration and pH of the solution. Have the dishwasher serviced and use the 3 compartment sink.
- Observed several foods in the walk-in cooler and the reach in cooler that were above 41 F. Foods were not actively cooling or being prepped. Ambient of the unit was currently 40 F however prior documentation shows that the unit sits in the 42-43 F range. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above. Recommended to person in charge to discard the food items and have the unit repaired.
- Observed foods that were in the act of cooling there were on the serving line. Per person in charge, the foods were prepped between 9 a.m. and 10:30 a.m. Time/temperature control for safety food shall be cooled within 4 hours to 41°F (5°C) or less if prepared from ingredients at ambient temperature, such as reconstituted foods and canned tuna. Foods were discarded.
- Observed several employee's drinks stored next to food for consumers in the reach-in cooler. Areas designated for employees to eat, drink, and use tobacco shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. Recommended to person in charge to retrain employees on designated areas.
