Observations: PIC not performing duties to ensure food safety. Observed hand washing violations and separation and protection violations. PIC should perform duties to ensure food safety. Observed cook line employee put on gloves without washing hands. PIC grabbed an herb garnish and placed it on top of prepared rice to be served with bare hands. Food was discarded.Multiple employee drinks in a single-use cup with a lid and a straw stored on prep tables near food, equipment and utensils. Facility serving house mayonnaise on sandwiches but could not provide receipt for mayonnaise. Observed cook line employee handle raw beef and then without changing gloves handled onions and peppers. Containers of onions and peppers were discarded. Unwashed vegetables above ready to eat food in walk-in cooler. Raw beef on top of house mayonnaise in walk-in cooler. Chicken stored above beef in walk-in freezer.Cooked meat in walk-in cooler stored uncovered. Multiple utensils stored clean had food debris remaining on them. Meat slicer had old food debris remaining on them. Ice machine had mold like build-up inside of unit. Multiple items (tapioca pearls, rice noodles and raw shell eggs) sitting out on prep tables not 41F or below. Multiple smoothie powders stored at drink counter not labeled with common name of food. Multiple food items in walk-in cooler and freezer stored on the floor. Multiple large pans stored on floor in kitchen. Multiple packages of cups stored on floor in dry storage area.Cook line employee took glove off and placed it on prep table then continued cooking. He then reused the same glove. Hand washing sink not draining properly. Lights near hand washing sink and lights near smoothie prep area not shielded.

Food stored in walk in cooler and freezer stored on the floor.Shrimps and other sea food items being prepped in vegetable sink. Food items removed from sink, sink washed rinsed, and sanitized. Vegetable sinks shall be for sole purpose of prepping fruits and vegetables.Several dishes still had sticky residues present on the outside of container. Dumpster pad had a lot of trash (boxes)on dumpster pad. Observed several live roaches, gnats, and flies in facility.Backdoor propped open.

Facility not abiding to time control procedures: Eggs were not discarded after 4hr holding time, Eggs were marked with 12pm discard time above grill at 1:38pm. Product discarded. Food items that were in process to be prepped were stored on the floor. Hash-browns and desert pies were stored in leaking cooler. Waffle batter buckets had sticky residue present on the outside of containers. Trash and debris on dumpster pad.

Ground beef and bison moved to walk-in cooler to cool down. Lemon butter near grill at room temperature (See temperatures below). Lemon butter discarded. Observed pans and utensils stacked wet. Pans moved to allow them to air dry.
