  • Observed multiple handwashing sinks that were not supplied with working soap dispensers. Must ensure all handwashing sinks are supplied with soap. Hand sanitizer is not a replacement/substitute for handwashing without soap. Must replace batteries in the non-working soap dispensers. New Violation.
  • Observed majority of food employees not wearing hair restraints. Employees preparing and/or handling food must wear hair net or other approved hair restraint (i.e. a McDonald's baseball cap) to restrain loose hair. This must be corrected. New Violation.
  • Partition that separates customers from McCafe area is not sufficient. There must be a glass, or other solid partition that protects area from customers and other environmental contamination. Must be at a minimum of 5' (feet) height from the floor. This is a repeat violation and must be corrected. Repeat Violation.
  • Observed several gaskets with buildup of food and debris. Must thoroughly clean gaskets and maintain the cleanliness of the gaskets. New Violation.
  • Observed front handsink (near french fry station) and 3-compartment sink profusely leaking. Must fix the leaking pipes and/or other parts that may be causing sinks to leak. New Violation.
  • Observed cardboard boxes on floor under drive-thru window (main-- where drinks and orders are handed to customers) that was said to be soaking up water from a leak. Do not use cardboard. Corrected on-site by discarding soiled cardboard and reattaching a part that was causing the leak. Corrected On-Site. New Violation.
  • Observed multiple open/exposed ceiling penetrations (drink lines in back of restaurant). Also, observed a missing ceiling vent cover which exposed large air duct. Must seal around ceiling penetrations with metal/caulking/foam caulking. This is a repeat violation and must be corrected. Must replace ceiling vent to cover exposed air duct. Repeat Violation.

  • Observed onions stored directly on floor. CA: food shall be protected from contamination by storing the food at least 6 inches off the floor. COS: Moved off the floor. Corrected On-Site. New Violation.
  • Observed wet wiping cloths out of the sanitizer. CA: Cloths in-use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n). COS: Moved to 50 ppm sanitizer. Corrected On-Site. New Violation.
  • Observed ice scoop handle stored in direct contact with ice. CA: In food that is not time/temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar, flour, or cinnamon. COS: Handle moved out of ice. Corrected On-Site. New Violation.

  • Observed employee take previously used cups from customers and reuse their cups for their next smoothie. Employee proceeded to take a chocolate sauce from the Reach-in Cooler (RIC) and spoon chocolate sauce in each customer's cup (there was direct contact with the sppon and the inside of the customer;s previously used cup). Health Authority instructed person in charge (PIC) to stop and explained proper procedure to reduce contamination which was to use a new, clean and unused cup for each new smoothie. PIC corrected on site by making new smoothies for each customer and also discarded chocolate sauce since it was in contact with each customer's used cup. Must not reuse customer cups for a new order. Must use new, clean, and unused cups each order. Corrected On-Site. New Violation.
  • Observed employee prepare several smoothies/shakes without wearing hair restraint. Must wear hair restraint while preparing foods (smoothies/shakes). New Violation.
  • Observed no handwashing signage at any of the handsinks. Handwash signs are required at all handsinks. Corrected on site by placing signs at handsinks (Health Authority provided signs). Corrected On-Site. New Violation.

  • Men's restroom sink does not have hot water. Water temp @ handsinks must be 100°F minimum. New Violation.
  • Facility does not have response procedure for illness incidents. Guidance will be provided by this office. New Violation.
  • Salad toppings(croutons, chips) on shelf not covered. Items were covered on request. Corrected On-Site. Repeat Violation.
  • Dish machine chlorine concentration at ~10ppm cl and must be 50 ppmcl. Call service for machine and use 3-comp sink until machine is dispensing sanitizer correctly. New Violation.
  • Most of the food storage containers had film of grease and food. It does not appear that dishware is being properly cleaned and sanitized. This must be corrected. Meat slicer, can opener, and ice machine interior have residue of food and scale that is a result of lack of or improper cleaning. Food contact surfaces must be properly cleaned/sanitized. New Violation.
  • Several made in house items(chicken wings, pasta) not date-marked. Items were prepared days ago. Items were discarded. Corrected On-Site. Repeat Violation.
  • Open ricotta cheese, mozzarella cheese, deli ham not marked with date items were opened. Items were dated on request. Date mark items as required. Corrected On-Site. Repeat Violation.
  • Sausage broth in walk-in cooler with date of 12-11-18. Broth was discarded. Corrected On-Site. New Violation.
  • Bulk seasonings not labeled. Label bulk items such as sugar and salt, when removing from original container. Repeat Violation.
  • Employee preparing food without hair restraint. Employees must wear hair restraints when preparing food. New Violation.
  • Current inspection report from 12/18, which was a 71/C was not posted (a previous score of 95 was posted). Current inspection report must be posted. New Violation.
  • Broken cooler gaskets must be replaced. New Violation.
  • Cutting boards are heavily scored and stained. Replace or resurface. New Violation.
  • No test strips for dish machine available. No test strips for quat sanitizer tablets available(prepared mixture was well over 500ppm). Facility must obtain test strips. New Violation.
  • Cleaning of equipment throughout kitchen is unsatisfactory. Grease/food buildup on most equipment. This must be addressed. Repeat Violation.
