Observations and corrective actions:

• Violation of Code: [.03(5)(c) ] Observed employee handle raw meats and then go and rinse hands in hand washing sink. Touch hat and apron and then put on gloves and work with food and touch clean equipment. Had employee remove gloves and wash hands again using soap and warm running water. Hands should be washed any time a risk of contamination is presented.  Corrected On-Site.  New Violation.
• Violation of Code: [.04(4)(a) ] Observed employee prepping onions and mushrooms for to-go soups with bare hands. Had employee discard onions and mushrooms. A suitable barrier should be used between bare hands and ready-to-eat foods.  Corrected On-Site.  New Violation.

• Violation of Code: [.05(7)(a)1 ] MOLD WAS INSIDE OF THE ICE MACHINE ICE BIN. CLEAN THE INSIDE OF THE ICE MACHINE. THE INSIDE OF THE ICE MACHINE ICE BIN WAS CLEANED.  Corrected On-Site.  New Violation.
• Violation of Code: [.07(2)(i) ] LIGHT SHIELD IS MISSING FOR LIGHT BULB AT TEACHERS' BUFFET. INSTALL A SOLID SHIELD OVER THE LIGHT BULB.  New Violation.
• Violation of Code: [.07(3)(g) ] EXHAUST FAN FOR THE RESTROOM IS NOT WORKING TO ELIMINATE ODORS. REPAIR OR REPLACE EXHAUST FAN.  New Violation.
