Bread was discarded and employee washed hands and donned new gloves.
• Observed an accumulation of black residue inside the ice bin located within the main kitchen. Equipment food-contact surfaces and utensils must be clean to sight and touch. Advised facility manager to monitor and clean ice bin frequently to avoid the buildup of residue. Replace Ice baffle if not black substance can not be removed.
• Observed multiple TCS food items for cold hold failing to maintain proper cold hold temperatures. (CA): All TCS food items for cold holding must be held at 41F or below. COS: Food items discarded.
• Observed 2 bottles of blue liquid stored on cart with know label or name on it. Working Containers, Common Name. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. Write name of chemical on all bottles.
• Observed Missing thermometers in units. All units must have thermometers to measure ambient air of unit.
• Observed ice scoop handle in contact with ice. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: in the food with their handles above the top of the food and the container. Scoop was re-adjusted.
• Observed broken door on reach in cooler. Door is off the hinges and falls off when attempt to open. Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. Fix unit today.
• All items out of temperature was discarded. New items placed on line with ice baths. Units need to be served to properly hold temperatures.Legal notice issued. Follow up to be conducted within 10 business days.
• A follow-up inspection will be held Jan. 1, 2019.

• Observed food container and strainer stored in hand sink near grill. Hand sink must remain accessible at all times.
• No hand soap observed at hand sink near sushi prep area. Advised PIC that soap must be provided to properly wash hands. COS: soap was provided.
• No paper towels provided at hand sink near soup and white rice. Advised PIC that paper towels. COS: paper towels were provided.
• Observed in several units raw animal foods stored next to ready to eat foods. Advised PIC on the proper way to separate and store foods. COS: food was located and stored properly.
• Employee observed using single use gloves to pick up shrimp from floor. Employee used the same gloves to prepare sushi. Informed PIC of proper glove usage. COS: sushi and gloves were discarded.
• Observed employee wash dishes without using soap. Advised PIC that a cleaning agent is needed. Advised PIC of the proper way to wash dishes wash-rinse- sanitize. COS: cleaning agent was added to the first sink.
• Observed employee on prep line clean knife at hand sink without properly washing or sanitizing. Advised PIC of proper ways to wash and sanitize utensils. COS: knife was cleaned and sanitized properly.
• Microwave observed soiled. Advised PIC that microwave must be kept clean due to it is a food contact surface. Advised PIC to clean microwave frequently. COS: microwave was cleaned.
• Did not observed time marking for sushi rice. Advised PIC of that sushi rice must be time marked and discarded after 4 hours. COS: time mark was provided.
• Observed wiping cloths stored on the prep counters. Advised PIC that all wiping cloths not in use must be placed in an active sanitizing solution. (COS): Placed in solution.
• A follow-up inspection is planned for Jan. 4, 2019.

• Observed Multiple containers of soy sauce stored on floor in kitchen area. Food shall be protected from contamination by storing the food: in a clean, dry location; where it is not exposed to splash, dust, or other contamination; and at least 6 inches (15 cm) above the floor. Food in packages and working containers may be stored less than 6 inches (15 cm) above the floor on case lot handling equipment if the equipment can be moved by hand or by conveniently available apparatuses such as hand trucks and forklifts.
• Observed knife stored in between prep unit and table. All utensils shall be stored in a manner where it is safe from contamination.
• Observed no test strips at facility. Facility must have test strips to properly test sanitizer solution. get test strips today.
• Observed excessive grease build up on the hood vents and on equipment in kitchen area. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Clean vents and equipment today.
• Observed floor drain by ice machine slowing/poorly draining. A plumbing system shall be repaired according to law and maintained in good repair. Call plumber today.
