• Observed a container of raw chicken stored above an uncovered container of onions in main kitchen walk-in cooler/improper vertical storage. CA: Items were stored in the correct order (cos). Items must be stored in the correct, descending order based on each items required final cook temperature.
• Observed build up of pink mold-like substance along interior of ice machine. CA: The ice maker must be cleaned at a frequency necessary to preclude accumulation of soil or mold. Advised that ice currently stored be burned and that the inside of the ice machine be cleaned.
• Observed dish machine failing to dispense chlorine sanitizer at a concentration between 50 and 100 parts per million. CA: Solution used was replaced with a generic brand of chlorine chemicals and sanitizing solution was restored to accessible levels.
• Observed items stored in cold holding units throughout main kitchen area failing to maintain temperatures of 41 degrees F or lower (see "temp. observations"). CA: All items were discarded onsite. Maintenance should be performed on units to ensure that cold holding temperatures are adequate.
• A follow-up inspection will be held Dec. 3.

• Observed utensil stored in employee hand sink located across from 3 compartment sink in the main kitchen area. Employee hand sink must be used for handwashing only and must be accessible at all times during operation.
• Observed failure to provide paper towels at employee hand sink in main kitchen located across from 3 compartment sink. Employee hand sink must be stocked with the proper hand drying provision at all times of operation. COS. PIC replaced disposable towels. Advised PIC to restock paper towels frequently.
• Observed heavy debris on can opener blade holster and directly on can opener blade. Advised PIC to clean frequently and routinely to eliminate build up on both items. COS. PIC properly cleaned can opener blade and holster during the time of the inspection.
• Observed pounds of meat being cooked at once on small grill in main grill area, once stated items were ready temperature was checked at 127F for raw meat. Advised PIC that beef must be cooked to 155 or higher. Items replaced on grill area for proper cook time of 165. Advised PIC to cook in smaller batches because of how small grill area is.
• Observed failure to have TCS food items in the walk in cooler at 41 F or lower during the time of the inspection (Chicken raw 47, Chicken Legs 57, Chopped chicken 50). Items properly discarded that were held over 24 hours and not actively prepared during the time of the inspection.
• Observed failure to properly cool TCS food items stored near grill area. Advised PIC to use approved cooling method such as ice bath or walk in cooler to properly cool TCS food items. Can not be left to cool at room temperature. PIC created ice bath for all cooling items found stored out on counters.
• Observed facility cooking a large amount of hot holding food items with no place to properly hot hold food items. Only small steam well near grill area. Advised PIC to cook in small batches to properly be able to hot hold items or purchase more hot holding items in facility. Equipment for cooling and heating food, and holding cold and hot food, shall be sufficient in number and capacity to provide food temperatures as specified under DPH Rule 511-6-1-.04.
• Observed failure to provide employee hand washing signage inside of men's restroom area. Must provide employee hand signage in all restrooms used by employees.
• A follow-up inspection held Nov. 21 resulted in the restaurant receiving a 96, or A.

• Observed shredded cheese cold held at 45°F. All time/temperature control for safety food shall be maintained at 41°F (5°C) or below. COS: Cheese was discarded.
• Observed boxes of noodles and coffee stored on floor. Food shall be protected from contamination by storing the food at least 6 inches off the floor. C/A: Advised PIC to remove food items from the ground.
• A follow-up inspection will be held Nov. 27.

Employee rewashed hand using proper procedure.
• Observed failure to have soap and paper towels at main kitchen hand sink and front food service hand sink. CA: All hand sinks must be stocked with soap and paper towels.
• Observed failure to have sanitizing solution readily available for wiping cloths. CA: Chemical sanitizer must be available. Facility must obtain.
• Observed ready-to-eat, potentially hazardous food prepared on site and held more than 24 hours with not properly date marked. CA: Items must be properly date marked and may only be held for seven (7) days. Items found improperly date marked were discarded onsite.
• Observed severe grease accumulation on hood ventilation system. CA: Ventilation hood systems and devices shall be sufficient in number and capacity to prevent grease or condensation from collecting on walls and ceilings. Must clean and repair hood.
• A follow-up inspection will be held Dec. 4.
