"Transitional care," according to Wikipedia, generally involves patients with a chronic or acute illness. It refers to the actions of healthcare providers that are designed to ensure the coordination and continuity of health care during the movement between health care practitioners and settings as the patients'Β  condition and care needs change.

This would mark another positive shot in the arm for development in the Suwanee Gateway. that has become an eyesore and crime nest; plans are to demolish the building.
