• Dispensing system at 3-compartment sink broken. Sanitizer concentration >500 ppm quat when tested. Solution was diluted to 300 ppm quat. Ensure sanitizer concentration is between 200-400 ppm quat. Dispensing equipment needs service.
• Employees preparing food without wearing hair restraints. Hair restraints must be worn when preparing food.
• Reach-in unit (pizza) has leak (interior). This must be corrected.
• Exteriors of squeeze bottles have product buildup and must be cleaned. Shelving for clean dishware has product buildup and must be cleaned.
• Faucet leaking at 3-comp sink. Cold water faucet not working at back hand sink. This must be corrected.

• Observed employees switching from tasks and changing gloves without washing hands. Observed employees wearing blue gloves over white gloves while handling raw meat, but they are not changing the gloves. They are discarding the blue gloves and using the same white gloves. Corrected onsite by discarding the gloves, washed hands and put on a new gloves.
• Everything in the kitchen must be stored 6" off the floor.
• Must wear beard restraints. Observed 2 employees with long beards.
