109 Gives Health Exam Deadlines
Health, dental and eye examinations are all required for some students.

Summer is an opportunity for students to obtain necessary medical examinations before the school year starts next month, according to information on the District website.
All entering kindergartners and students new to Illinois must have a health, dental and eye exam. The medical and eye examination must be done within the last year and given to the appropriate school by Oct. 15. Eyes can be checked by either an optometrist or a medical doctor who gives vision tests.
The dental exam must be completed by May 15 of the current school year by a licensed dentist within 18 months of the due date, according to the information on the website. Entering second graders must also complete this requirement.

Students entering sixth grade are required to have both a health and dental exam. The same requirements apply.
All middle school students participating in athletics must have a health exam; submit a signed parental permission slip and a signed receipt of concussion injury materials, according to the website. Sixth graders who obtain the regular physical may use the same form for sports.
