Lake Forest Seeking Public Service Award Nominations
he award recognizes a living volunteer in community organizations or local government for distinguished public service.

Nominations are now being accepted for the Lawrence R. Temple Distinguished Public Service Award established in honor of the late Second Ward Alderman Larry Temple and his contributions to Lake Forest. The award recognizes a living volunteer in community organizations or local government for distinguished public service. Past recipients of the Temple Distinguished Public Service Award are former mayors Francis C. Farwell, Rhett Butler and James Swarthout, and former Alderman Herbert Faulks.
The deadline for submitting award nominations is Wednesday, October 1, 2014. Nomination forms are available at the Lake Forest City Hall (220 E. Deerpath) and online at www.cityoflakeforest.com in the News section.

Completed nomination forms for the Temple Distinguished Public Service Award Committee should be mailed to the Lake Forest City Hall, 220 East Deerpath, Lake Forest, Illinois 60045 or deposited in one of the City’s white drive-up boxes conveniently located in the East Train Station parking lot just off Deerpath, in the Telegraph Road Station parking lot and on Bank Lane north of Westminster next to the U.S. Post Office boxes.
