Take The WCHD Survey To Sign Up For COVID-19 Vaccination

The survey is only for Will County residents who wish to get the COVID-19 vaccine.

WILL COUNTY, IL — To prepare for the distribution of the COVID-19 vaccine, the Will County Health Department is asking residents to take a survey.

Questions contained in the survey were developed from limited and preliminary guidance from the Center for Disease Control and Prevention and the Advisory Committee on Immunization Practices. County residents will be asked to provide personal information such as address, contact number and employment status.

Other questions in the survey pertain to assessing whether you are a health care/ front line worker, if you have pre-existing health conditions and if you live or work in a long-term care facility.

Access the survey here. This will ensure you are signed up for when the vaccine is available.

A total of 4,960 doses of the Pfizer coronavirus vaccine arrived Dec. 16 at the Will County Health Department in Joliet and the doses were delivered to four hospitals across the county to AMITA Health Saint Joseph Medical Center in Joliet, Silver Oaks Behavioral Hospital in New Lenox, Silver Cross Hospital and AMITA Health Adventist Medical Center in Bolingbrook.

The Vaccination Campaign will be a phased approach per CDC guidance. There will be a limited supply of COVID-19 vaccines in December, but supply will continually increase in the weeks and months that follow, the WCHD website states.

The long-term goal is for everyone to be able to easily get a COVID-19 vaccination as soon as large quantities are available. The plan is to have several thousand vaccination providers available, including doctors’ offices, retail pharmacies, hospitals, and federally qualified health centers.

COVID-19 Phase 1 Vaccine Distribution

Phase 1 of distribution is when there is limited/scarce supply of COVID-19 vaccine doses available. Focus initial efforts on reaching critical populations. Ensure vaccination locations selected can reach populations, manage cold chain requirements, and meet reporting requirements for vaccine supply and uptake. Vaccine administration strategies in phase 1(a), will begin with vaccine administration for:

  • Health Care Personnel
  • Long Term Care Facility Staff and Residents
