Most Everyone Qualifies for a Booster ShotWe will be providing MODERNA Booster Shots,to qualify, it must have been at least five months sinceyour second dose of Moderna or Pfizer vaccineor two months since your J & J shot.

NOTE: You can also register through the links belowfor primary series (1st and 2nd dose) Moderna shots.

Due to COVID-19 protocols, please arrive at the time of your appointment to minimize the numberof people waiting and wear a face-covering

*Please wear a short sleeve shirt and bring your VACCINE CARD*

Reminder: 15-minute post-vaccination monitoring is required.
