The clinic is for adults 19 and over from all communities and will run from 9 a.m. to 12 p.m.Attendees are asked to wear a face covering and short sleeves and bring:<ul><li>A completed <a href="https://www.wakefield.ma.us/health-department/files/2020-10-17-regular-dose-flu-clinic-form" target="_blank" rel="nofollow">regular vaccine form</a> or <a href="https://www.wakefield.ma.us/health-department/files/2020-10-17-high-dose-flu-clinic-form" target="_blank" rel="nofollow">high-dose vaccine form</a>, depending on your preference, and</li><li>Your insurance card(s) </li></ul>The vaccine is free with most insurances except Aetna and United Healthcare. There will be a $15 fee for regular flu vaccine not covered by a participating insurance carrier. A limited amount of high-dose vaccine is available and out-of-pocket payment is not an option for this type; we are only accepting insurance for the high-dose vaccine.If you have questions, contact the Health Department at (781) 246-6375.
