ADDENDUM #5 

CONSTRUCTION MANAGEMENT PACKAGE - Bidding Requirements & General Conditions

BID PROPOSAL EXTENSION DEADLINE: TUESDAY, OCTOBER 26, 2021 @ 1:00 P.M. (local time) A. Section 00 21 13 - Instructions to Bidders 

1. Bids for BD 109-Membrane Roofing ONLY have been extended. Bids are due on  Tuesday, October 26, 2021 @ 1:00 p.m. (local time). Late bids will not be accepted.

2. New ZOOM link for Re-Bid can be found here: https://us02web.zoom.us/j/89380469708

3. Apparent low, qualified bidders will be contacted for a post bid interview. Post bid  interviews will take place virtually.  

4. All bids are to be accompanied by a Bid Security in the amount of five (5) percent of the  amount of the bid as a guarantee that if the proposal is accepted, the bidder will execute  the contract and file the required bonds within ten (10) days after notice of award of  contract. 

5. All bids are to be accompanied with a signed and notarized Affidavit of Bidder form for the  Iran Economic Sanctions Act. This form is available in Section 004210 of the Bidding  Requirements & General Conditions Manual.
