Centrex Rehab Dissolving Operations, Layoffs To Hit 311 Minnesota Workers
More than 300 employees at Centrex Rehab just learned their jobs are ending this fall, state records show.
BLOOMINGTON, MN — Centrex Rehab will permanently close and lay off approximately 311 employees this fall, according to a federal WARN notice filed with the State of Minnesota.
Centrex informed the State Rapid Response Team that it has decided to dissolve its operations. Employees will experience permanent layoffs effective Nov. 7, 2025.
The layoffs impact a wide range of positions, including Clinical COTA, Clinical OT, COTA, HR, Insurance Verification, OT/OT/RD, PT, PT/RD, PTA, PTA/RD, RA, Regional OT, SLP, SLP/RD, Sr Ops/PT, and Staffing.

Employees are not represented by a union, and no bumping rights exist, the notice states. Centrex said it is actively working to identify and secure employment opportunities for the majority of affected staff.
According to its website, Centrex Rehab highlighted its "advantage" as a therapy provider by focusing on quality care, employee collaboration, and clinical support. The company described itself as offering therapy solutions across skilled nursing communities, senior housing, assisted living, clinics, and home health locations in multiple states.

The Minnesota State Rapid Response Team assigned Sr. Specialist John Mohs to lead the assessment of the closure.
