Northwell Names New Medical Director Of House Calls Program

NEW HYDE PARK, NY — Northwell Health named Dr. Konstantinos Deligiannidis as medical director of its House Calls program, the healthcare network announced in a Tuesday press release.

In his new role, Deligiannidis will provide direct care and supervise a multidisciplinary staff to deliver health care to elderly patients in their homes in Long Island, Queens, and Manhattan, the press release read.

"Dr. Deligiannidis brings strong leadership and collaboration among colleagues along with honed clinical skills in treating patients at home with multiple chronic illnesses to deliver the best possible care," said Dr. Zenobia Brown, medical director and vice president of population health care management at Northwell Health Solutions.

Deligiannidis will assume his new position after serving as House Calls' director of education and quality since 2016.

Billed as New York's largest healthcare provider, Northwell is headquartered in New Hyde Park. It boasts 23 hospitals, 830 outpatient facilities, and more than 16,600 affiliated physicians. The health system's program, House Calls, offers patients coordinated care — similar to treatments and referrals they would receive from a primary care physician — such as ultrasounds, prescription refills, and lab work.

Prior to joining Northwell in 2016, Deligiannidis was a family medicine physician at UMass Memorial Health Care in Barre, Massachusetts. He earned his bachelor's degree at Harvard University, his Master's in Public Health at Boston University, and his Doctor of Medicine at the University of Massachusetts Medical School.

In 2018, Deligiannidis was named House Call Educator of the Year by the American Academy of Home Care Medicine.

He is currently an associate professor in medicine at the Donald and Barbara Zucker School of Medicine at Hofstra University.
