Helping Upper East Siders Age Gracefully at Home
Pharmacist Ilana Yagudayev helps older adults stay healthy, independent, and supported in the comfort of their own homes.

On the Upper East Side, community pharmacist Ilana Yagudayev has become a trusted name synonymous with compassionate care and healthy aging. As the owner of Drug Mart Pharmacy, on the corner of 86th St. and York Ave, Ilana has spent more than two decades helping neighbors feel their best—through personalized care, education, and a deep commitment to well-being.
Her latest initiative, Age Gracefully at Home, was born from a growing need she saw in her patients: seniors who wanted to stay home safely after hospital or rehab stays, but who struggled to manage medications and coordinate care.
“Many older adults are happier and heal faster when they’re at home,” Ilana says. “But without the right support, it can become stressful for families. That’s where we come in.”
Through the Age Gracefully at Home program, Ilana and her team partner with doctors, nurses, and caregivers to deliver pharmacy-based, in-home support services that help older adults thrive in place.

- At-home immunizations — making vaccinations accessible, convenient, and safe for homebound patients
- Diabetes education and support — empowering individuals to manage their blood sugar, diet, and medications with confidence
- Education and wellness guidance — helping families make informed, long-term health decisions
Unlike traditional pharmacies, Ilana’s approach centers around building long-term relationships and understanding each patient’s lifestyle, not just their prescriptions.
“My goal is to simplify care and reduce unnecessary medications,” Ilana explains. “When patients are informed and supported, they feel better, stay active, and enjoy life longer.”
Beyond her pharmacy, Ilana regularly volunteers at local senior centers, offers free community health fairs, and collaborates with area doctors to host wellness workshops on topics like medication safety, diabetes prevention, and natural healing.
Ilana believes healthcare should feel personal — not transactional. “Pharmacy is about people,” she says. “Our mission is to help our neighbors live fully, independently, and age gracefully at home.”
