A witness told deputies a black SUV was attempting to pass a vehicle on Mulino Road when it collided with a red sedan approaching in the oncoming lane.

The driver of the black SUV, age 17, was transported to a local hospital with unknown injuries.

The red sedan had three occupants, ages 10 - 16. Their conditions:

  • Driver,16, was declared deceased at the scene.
  • Two passengers age 12 and 10 were transported to a local hospital with unknown injuries.
  • The Clackamas County Medical Examiner's Office and the Criminal Reconstruction and Forensic Technicians (CRAFT) team also responded to document the scene, conduct witness interviews and collect and process evidence. Clackamas Road Department and Canby Police assisted with traffic control.

    This incident is still under investigation.
