Arlington Restaurant Inspections: Droppings, Molded Fruit, No Labels
Arlington County Public Health recorded violations at several restaurants around Arlington during inspections between Aug. 17 and Aug. 31.
ARLINGTON, VA — Several facilities in Arlington recorded violations during recent inspections from the Arlington County Public Health department. Inspectors conducted the most recent visits between Aug. 17 and Aug. 31.
In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.
The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."

- Observed that dish machine is not dispensing sanitizer. Corrective Actions: Adjust the chlorine sanitizing solution in the dish machine to a level of at least 50 parts per million. Until the dish machine has been repaired, please wash rinse and sanitize in the 3 compartment sink.
- Dead or trapped pests were found on the premises on equipment gaskets. Corrective Actions: Remove dead or trapped birds, insects, rodents, and other pests from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests. Clean and check the gaskets of the reach in freezer thoroughly. PIC cleaned area during inspection.

- Back kitchen hand sink not in use; ensure all hand sinks have individually dispensed towels. Corrective Actions: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel, heated air hand drying device, or a hand drying device that employs an air-knife system that delivers high-velocity, pressurized air at ambient temperature must be provided at all handwashing sinks to encourage proper handwashing and avoid employees to drying their hands on their clothing or other unclean materials.
- Dish machine found not ejecting 50-150 ppm of chlorine; don't use until repaired to do so, use 3-part sink. Corrective Actions: Immediately discontinue the use of the machine until it is operating in accordance with minimum requirements. Initiate manual wash, rinse, and sanitize method to properly clean and sanitize equipment and utensils. To minimize workload you can move to all single service utensils.
- Black mold inside ice machine at the bar and pink mold in kitchen ice machine. Corrective Actions: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.
- Some sprouts stored above ice improperly cold holding up to 60 F--> completely surround with ice or place in drained ice (container with holes so when ice melts, can drain into a container below) at improper temperatures. Corrective Actions: Ensure that time/temperature control for safety (TCS) foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria.
- Not all chemical bottles are labeled with the common name of product. Corrective Actions: Properly label working containers of toxic items with the common name.
- "Raid" ant & roach spray for "household use" not allowed in the facility. Corrective Actions: Restricted used pesticides are to be applied by an applicator certified as defined in 7 USC 136(e) Certified Applicator, of the Federal Insecticide, Rodenticide Act, or a person under the direct supervision of a certified applicator.
- Garbage disposal back flow prevention device by the dish machine is no longer in good repair. Corrective Actions: Plumbing systems and components shall be maintained in good repair.

- Observed that soups batches of lasagna were not dates and take more than 24 hours to serve the entire portion. Corrective Actions: Label all foods that take longer than 1 day to serve.
- Observed that mops are left damp and heavily soiled, and are attracting gnats/drain flies.Corrective Actions: Ensure facilities are maintained to prevent harborage conditions for pests, and that mops are thoroughly cleaned before drying and walls and floors are kept clean and dry.

- Found can dented on its side seam on shelving; removed from shelving. Corrective Actions: Discard or return food products once the packaging is damaged or is in bad condition. Food packages must be in good condition to protect the integrity of the contents so that food is not exposed to adulteration or potential contaminants.
- Bar dish machine only sanitizing at 25 ppm of chlorine; discontinue use until 50-150 ppm. Some mold inside the ice machine; drain, clean and sanitize. Corrective Actions: Provide chlorine at proper concentration of 50-150 ppm and immerse or expose food contact surfaces to sanitizing solution for adequate time.
- Containers of sushi rice, trays of sushi rolls, tray of tempura shrimp, bowl of asparagus, container of tofu pieces at server prep table aren't time marked to be used/discarded within four hours from the time it was prepared and/or removed from temperature control. Corrective Actions: Discard time/temperature control for safety (TCS) food that is not cooked and served or served if ready-to-eat within 4 hours from the point in time when the food is removed from temperature control. Clearly label or otherwise mark the food with a 4-hour cook-serve or serve by time.
- Roach (German) observed on floor by walk-in refrigerator. Corrective Actions: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.

Corrective Actions: Immediately discontinue the use of the machine until it is operating in accordance with minimum requirements. Initiate manual wash, rinse, and sanitize method to properly clean and sanitize equipment and utensils.
- Juicer not clean. Corrective Actions: Clean the surface of @EQUIPMENT@ at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.2
- Cooked chicken stacked too high in the top portion of the "right 3-door prep refrigerator" found improperly cold holding up to 57 F on top; chicken rearranged inside the unit. Corrective Actions: Cold hold time/temperature control for safety foods at 41 F or below to inhibit the growth of harmful bacteria.

- Foods in the front counter unit improperly cold holding at 51 F; manager stated placed out at 11 a.m.; discard any leftovers by 3 p.m. A risk control plan will be emailed to the owner about this repeat violation. - Sink & surface sanitizer level found too strong (> 700 ppm) at 91 F; corrected on-site by draining and refilling, concentration lowered to 272 ppm. Corrective Actions: Use chemical sanitizers that meet the requirements of 40 CFR 180.940 or 40 CFR 180.2020 when applying to food contact surfaces.
- Fruit flies and cockroaches observed in back kitchen; eliminate their presence as well as birds in the terminal. Corrective Actions: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.

- French fries 70 F by fryer not time marked, "Time As Public Health" control written procedures not available. I emailed the facility written procedures for them to utilize. Corrective Actions: If time only, rather than time in accordance with temperature control, up to a maximum of 4 hours is used as the public health control : 1) the food shall have an initial temperature of 41°F or less if removed from cold holding temperature or, 2) the food shall have an initial temperature of 135°F or above if removed from hot holding temperature control, 3) the food shall be marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control, 4) the food shall be cooked and served, served if ready to eat, or discarded within 4 hours from the point in time when the food is removed from temperature control, and 5) the food if unmarked or marked to exceed a 4 hour limit shall be discarded.

- Obtain a food thermometer that can take temperatures 0-220 F in two degree increments. Corrective Actions: Provide a food temperature measuring device with a numerical scale, printed record, or digital readout in increments no greater than 2°F or 1°C in the intended range of use.
- Properly re-label containers used to store salt, sugar, etc. Corrective Actions: Label working containers with the common name of its contents. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning.3
- Mall loading dock doors aren't self-closing or tight-fitting; found open. Corrective Actions: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors.
- Don't use cloths under cutting boards. Corrective Actions: Non-food contact surfaces of equipment that are exposed to splash, spillage, or other foods that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.
- Mall trash & recycling containers aren't kept closed with tight fitting lids. Corrective Actions: Keep receptacles and waste handling units for refuse, recyclables, and returnables kept outside the food establishment covered with tight-fitting lids or doors.
- Mop sink drain and the floor drain under the grill are missing covers. There are some holes/openings in the walls and ceiling. Corrective Actions: Replace the missing drain covers. Fill in any holes or openings in the ceiling and walls.
- The hood is missing a baffled vent, drip pan and the steel walls aren't sealed/caulked closed at the electrical outlet, top , bottom and corner panels. Corrective Actions: Replace the missing baffle. Provide a drip pan. Seal/caulk up the steel panels to at their adjoining walls. Alter or replace the hood system to prevent grease or condensation from draining or dripping onto food, equipment, utensils, linens, single-service, and single-use articles.

Corrective Actions: Adjust the walk in cooler so that it is able to maintain foods at 41F or below during peak use, and ensure temperature control for safety foods are not held on the countertop for extended periods. Cooler was adjusted to an ambient temperature of 38 during inspection and cream was placed back in the cooler.
- During inspection observed two containers of consumer grade sprays in the establishment. Corrective Actions: Restaurant staff are not permitted to use pest control sprays in food establishments, as improper application may result in the contamination of food equipment and prep surfaces. Restricted used pesticides are to be applied by an applicator certified as defined in 7 USC 136(e) Certified Applicator, of the Federal Insecticide, Rodenticide Act, or a person under the direct supervision of a certified applicator.

A longer time for cooling allows an ideal situation for bacterial growth. This has been shown to be the major contributing factor in many foodborne illnesses. Corrected by discarding.
- Foods (cheese, cooked spinach) found improperly cold holding at 45-46 F inside the 2-door prep refrigerator. Properly store foods in the top portion of the unit (don't double stack containers). Re-adjust unit as needed to keep foods 41 F and below. Foods relocated in meantime to correct violation on-site. - Mouse droppings were observed on the kitchen floor under the shelving where drinks are stored. Fruit flies observed on coolers and walls by the ice machine.
