Falls Church Restaurant Inspections: 18 Violations At One Eatery

During restaurant inspections in September, one restaurant was found with drain flies and 17 other violations.

FALLS CHURCH, VA — The Fairfax County Health Department continues to inspect restaurants around Falls Church, and recent inspections show a mix of restaurants with violations and perfect reports.

The health department's environmental health staff inspect over 5,200 restaurants and other retail food service establishments in Fairfax County and cities of Falls Church and Fairfax to ensure safe food handling practices are followed. This includes proper sanitation, food storage and preparation, and adequate kitchen facilities. Aside from restaurants, staff inspect child care centers, hospitals, school cafeterias, and temporary food service vendors at festivals.

This week's report focuses on September inspections in the City of Falls Church. One noteworthy result was at Pho 95 Mi Cay Tung with 18 violations, including drain flies.

The health department cautions residents that a single inspection should not be used to evaluate a restaurant. Rather, the health department suggests looking at multiple inspection reports for a better overview.

Last inspection: Sept. 1

Violations:

  • EHS observed cooked rice (44F), pulled pork (45F), pico de gallo (43F), coleslaw (43F), cooked black beans (45F), and carnitas (45F) in walk-in cooler not being adequately cooled from 135F to 70F within two hours and to 41F within a total of six hours.
  • The following methods used for cooling were not adequate to facilitate proper cooling: EHS observed cooked chicken being cooled in walk-in cooler (107-112 Fat 1.5 hr).
  • Surface of ice bin is not being cleaned as required. Observed ice machine with buildup of mold and debris.

Tasty Dumpling: 1112 W Broad St., Falls Church, Va 22046

Jersey Mike's Sub: 1108 W Broad St., Unit# A, Falls Church, Va 22046

Violations:

  • Observed that quaternary ammonia tablets are stored above food prep surface and not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items.
  • The following methods used for cooling were not adequate to facilitate proper cooling: sliced tomatoes from ambient densely packed in walk-in. Tomatoes at 57F after 5 hours. Sliced tomatoes also observed cooling in cold well at 50F after 5 hours.

Chasin' Tails: 944 W Broad St., Falls Church, Va 22046

  • The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: chopped lettuce (45-49F), cooked rice (43F), bean sprouts (45F) in Roll Play 2 dr upright unit. Tofu (48F) in Roll Play cooling table. Tofu was placed within 3 hr.

  • It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager who has shown proficiency through passing an accredited examination. Observed PIC with expired CFM certificate.

Moby Dick House of Kabob: 444 W Broad St., Ste# A, Falls Church, Va 22046

Violations:

  • Employees' medicines are not properly located to prevent contamination of food and food contact surfaces. Observed over the counter medications being stored above food prep surfaces.
  • The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: cut tomato (44F), chopped lettuce (43-46F), falafel (43F), sliced cheese (45F) in 2 dr prep unit. All items were placed in unit 1 hr before inspection and came from WIC.
  • Observed that poisonous and toxic materials are not properly stored to prevent the contamination of single service items.degreaser and cleaning solutions observed above single service plates in dry storage area.

  • Restricted, commercial use pesticides present in the facility are applied by the operator who is not a certified pesticide applicator. Residential use ant and roachspray observed stored with chemicals under 3 compartment sink.
  • ROP tuna observed thawing in original sealed packaging in 3 dr flattop unit at raw bar.
  • Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the food prep areas of the kitchen and raw bar.

Belinda's Restaurant: 404 S Washington St., Falls Church, Va 22046

Violations:

  • Employees' medicines are not properly located to prevent contamination of food and food contact surfaces. EHS observed pill bottles stored above food preparation areas of the kitchen
  • The following cooked time/temperature control for safety food(s) are not being adequately cooled from 135F to 70F within two hours and to 41F within a total of six hours. Rice (52F), beans (60F), cooked chicken (58F) in 3 dr upright. Cut tomato, cheese, chopped lettuce (45F) in 2 dr prep unit.
  • The following methods used for cooling were not adequate to facilitate proper cooling: large sauce pot of beans observed at 60F in 3 dr upright. Large containerof cooked chicken observed at 58F in 3 dr upright. Cut tomato, cheese, and chopped lettuce observed in top of 2 dr prep packed heavily and with other items stacked on top in the wells.

Super Chicken: 422 S Washington St., Falls Church, Va 22046

Violations:

  • There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
  • Observed the handsink at the dish line being used to store strainer.
  • A food employee was observed eating, smoking, or drinking in areas where they may contaminate food, clean equipment, utensils or other items needing protection. Multiple employees observed eating at prep surface in back of kitchen.
  • Food stored on the floor and/or food stored less than 6 inches off the floor. Boxes of packaged raw beef, and French fries observed being stored on the ground.

El Patron Bar and Grill: 418 S Washington St., Falls Church, Va 22046

Violations:

  • Chemical sanitizer immersion time was not adequate for bleach sanitizing solution. EHS observed food service utensils being washed without use of sanitizer.
  • The permit holder failed to discontinue operations and notify the Health Department that an imminent health hazard may exist due to an active sewage back-upcoming from the three compartment sink drain.
  • Observed improper disposal of sewage. Observed active sewage backup coming out of the three compartment sink basin.

  • Raw animal food storage, preparation, holding or display conducted in a manner that may cause cross contamination of ready-to-eat food as follows: raw chicken stored above RTE sauces in 2 dr flattop refrigerator.
  • The following time/temperature control for safety food(s) are not being adequately cooled from 135F to 70F within two hours and to 41F within a total of six hours. Rice in 2 dr upright (50F), cheese in 1 dr upright (47F), chicken soup (50F) in 2 dr upright downstairs.

  • The following food item(s) were observed hot holding at improper temperatures using a calibrated food temperature measuring device: pork broth (131°F) and fish balls (115°F).
  • The following food item(s) for which time rather than temperature is used as a control is not marked or otherwise identified to indicate the time that is 4 hoursvpast the point in time when the food is removed from temperature control: 1) bean sprouts, 2) washed lettuce, 3) garlic in oil.
  • Methods are not being used to control pests: significant number of drain flies in kitchen.
  • The food establishment does not have written procedures for employees to follow when responding to vomiting or diarrheal events.
  • Observed that hand soap was not provided at each handwashing sink.
  • The certified food manager could not provide a food temperature measuring device.
  • A review of the menu with the foodservice operator indicates that there is no consumer advisory for the following food item(s) that may be served raw and/orundercooked: soups containing rare steak.
  • Observed that paper towels were not provided at each handwashing sink.
  • Observed a 15 KW electric water heater in the ceiling in the kitchen. Water heater is undersized for peak hot water demand.
  • The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41F: delfield 2 door under counter refrigerator, dukers 2door refrigerated prep unit and the walk in cooler we’re all observed at 45°F.
  • Observed that the floor around and behind equipment, plumbing under the 3 compartment sink and the walls in the kitchen are in need of cleaning.
  • Observed open water cups and coffee beverages in the kitchen areas.
  • Observed that sign or poster that notifies food employees to wash their hands is not provided or is not clearly visible at all handwashing sinks used by food employees.

Thanh Truc: 6795 Wilson Blvd., Falls Church, Va 22044

Violations:

  • Observed that poisonous and toxic materials are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items.
  • Observed that the floor around equipment is in need of cleaning.
  • The food-contact surfaces of the following equipment and/or utensils are not finished to have a smooth, cleanable surface: plastic bucket used to store sugar, plastic buckets in fridge (looked to be reused).
  • Observed onions and flour stored on the floor.
  • The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: shelving inside of fridge and freezer, interior of microwave,exterior of food storage containers.

Saigon Restaurant: 6795 Wilson Blvd., Unit# 25, Falls Church, Va 22044

  • The following food item(s) were observed hot holding at improper temperatures using a calibrated food temperature measuring device: shrimp (128°F), pork andegg (130°F), fish (109°F).

  • The following cooked time/temperature control for safety food(s) are not being adequately cooled from 135F to 70F within two hours and to 41F within a total of six hours. Observed tomato and crab meat soup in the Maxx Cold 2 door refrigerator made yesterday at 57°F.
  • The only hand washing sink in the kitchen is blocked preventing access by employees for easy handwashing.
  • The food establishment does not have written procedures for employees to follow when responding to vomiting or diarrheal events.
  • The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: the exterior of the fryer and of the chest freezer are in need of cleaning.

  • The nonfood-contact surface of the following equipment is not corrosion resistant, nonabsorbent, and/or smooth: observed top of chest freezer used to set cooling food covered in cardboard.
  • Observed that the wall behind the cook line, wall behind the three compartment sink, the wall by the mop sink, and the floor around equipment are in need ofcleaning.
  • The food-contact surfaces of the following equipment and/or utensils are not finished to have a smooth, cleanable surface: observed sugar being stored in HomeDepot buckets.
  • The following food items that are not easily identified by appearance were observed without a label: observed containers of white powders and sauces/oils not labeled.
