Introduction
Menopause is a natural but often challenging transition for many women, with symptoms that can disrupt daily life—but effective solutions are available. Perimenopause, typically starting in the mid-40s, signals the transition to menopause, defined as no period for one year. Hormonal changes during this time can lead to symptoms like hot flashes, sleep issues, and mood swings, but hormone replacement therapy (HRT) can provide relief. Dr. Shannon Cothran, an OB-GYN and Menopause Society Certified Practitioner (MSCP), founded Meno-Start in McLean, Virginia, to offer expert care and help women thrive during menopause, with a focus on HRT.
What is Hormone Replacement Therapy?
Hormone replacement therapy (HRT), also known as menopausal hormone therapy (MHT), helps stabilize hormone levels as the body experiences declining estrogen and progesterone during menopause. Estrogen supports bone density, cardiovascular health, and reproductive functions, while progesterone plays a vital role in protecting the uterus and reducing the risk of endometrial cancer in women who still have one. The primary goals of HRT are to regulate hormone levels, alleviate menopausal symptoms, promote bone and heart health, preserve the health of vaginal and urinary tract tissues, and improve overall well-being. “Hormone replacement therapy isn’t just about easing hot flashes—it's about restoring balance in your body and supporting long-term health,” said Dr. Cothran.
What Are the Basic Types of Hormone Therapy?
HRT is available in three main types, each tailored to different needs. Sequential Combined Therapy involves daily estrogen supplemented by progestogen for 12-14 days each month, mimicking the natural ovulation cycle, though it may not always cause bleeding. Continuous Combined Therapy, which includes daily estrogen and progestogen, is often recommended for women at least one year past their last menstrual period to control menstruation, though oral contraception is typically more effective for this purpose. For women who have undergone a hysterectomy, Estrogen-Only Therapy is an option, excluding progestogen, and can also be administered locally for vaginal treatment.
How is Hormone Therapy Delivered?
HRT can be delivered systemically or locally, depending on the symptoms being addressed. Systemic therapy delivers hormones into the bloodstream, benefiting multiple organs and tissues. Common forms include oral pills, nasal sprays, skin patches, creams, and gels, effective for managing symptoms like hot flashes and night sweats. Local therapy, on the other hand, targets specific areas, particularly the vagina. Using vaginal creams, rings, or tablets, this method effectively treats symptoms such as dryness and discomfort during intercourse.
Is hormone replacement therapy right for me?
HRT offers effective relief for many menopausal symptoms, including hot flashes, night sweats, mood swings, brain fog, and vaginal dryness. While HRT is generally safe for healthy women in the early stages of menopause, it’s not suitable for everyone. Factors such as smoking, certain cancers, or cardiovascular conditions may preclude its use. “Menopause is a unique journey for every woman, and there’s no one-size-fits-all solution. At Meno-Start, we take the time to understand your health history, lifestyle, and personal goals to explore the best options for managing your symptoms. My goal is to provide a customized plan that helps you feel your best during this transition,” said Dr. Cothran.
