Up To 20 Violations At 1 Eatery: Vienna Restaurant Inspections

Up to 20 violations or no violations were found during July's health inspections in Vienna.

VIENNA, VA — As the Fairfax County Health Department regularly inspects restaurants in the Town of Vienna, we continue to look at the latest inspection results.

The health department's environmental health staff inspect over 5,200 restaurants and other retail food service establishments in Fairfax County and cities of Falls Church and Fairfax to ensure safe food handling practices are followed. This includes proper sanitation, food storage and preparation, and adequate kitchen facilities. Aside from restaurants, staff inspect child care centers, hospitals, school cafeterias, and temporary food service vendors at festivals.

Residents can search for a specific restaurant or browse a map of inspected locations through the online database. The health department cautions residents that multiple inspection reports should be used to evaluate a restaurant rather than a single report.

This week's report focuses on reports in July around the Town of Vienna. Of note, Anita's New Mexican Style Food had 20 violations.

Violations:

  • There is no Certified Food Manager present at the beginning of the inspection.
  • Observed 2 working chemical spray bottles not properly labeled with a common name in the kitchen area.
  • Observed multiple wet wiping cloths used for wiping counters and other equipment surfaces stored on the counter throughout the kitchen area.
  • Observed that the floors under shelves inside of the walk in cooler is in need of cleaning.
  • Observed that clean metal sheet pans and other kitchen equipment were stored on shelf inside front of fan with excessive dust buildup in the kitchen area near the walk in cooler.
  • Observed metal pot of cooked potatoes stored directly on the floor inside of the walk in cooler.

Sushi Yoshi: 101 Church St, Vienna, Va 22180

Violations:

  • Pesticides labeled for residential use (raid) were observed in the facility and are being applied by the operator to control pests.
  • Raw animal food storage, preparation, holding or display conducted in a manner that may cause cross contamination of ready-to-eat food as follows: raw fish stored above uncovered RTE house inside of the reach in cooler on the cookline, raw yellowtail tuna stored above RTE sauces inside of the walk in cooler, uncooked yellowtail tuna and uncooked spicy tuna stored above RTE sauces inside of the 1 Dr flattop cooler.
  • The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Observed cooked marinated tomatoes stored inside of ice bath with temperature of 50F (top half).
  • Observed the food establishment does not have written procedures for employees to follow when responding to vomiting or diarrheal events.
  • Observed ice dumped in handsink at the end of the cookline.
  • Observed paper towels were not provided at handwashing sink at the end of the cookline.
  • Observed floor grout (low grout lines) is not maintained in good repair throughout kitchen area.
  • Observed to single-service items (to go containers and plastic serving boxes) stored in boxes directly on the floor in the basement.
  • Observed bag of radishes stored directly on floor in the kitchen area near 3 compartment sink.
  • In-use utensils improperly stored between use as follows: rice scoop stored in container of standing water 97, ice scoop directly touching ice inside of ice machine in kitchen area.

Lombarbi's Pizza: 111 Maple Ave, Vienna, Va 22180

Violations:

  • The certified food manager could not provide a food temperature measuring device.
  • Observed floor not coved to wall near the front Continental 2 Dr UP cooler.
  • Observed bulk bag of flour and box of tomatoes stored directly on the floor in the back kitchen area.

Peking Express of Vienna: 103 Center St, Vienna, Va 22180

Violations:

  • The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: accumulation of debris on shelves inside of walk in cooler
  • Observed debris on the interior of the ice machine in the kitchen area. Observed black mold-like debris on soda nozzle/ dispenser at the soda fountain machine in the front service area.
  • In-use utensils improperly stored between use as follows: Rice scoop stored in container of standing water with temperature of 83F in kitchen area. Rice scoop stored in container of standing water with temperature of 80F in the front service area.

Ledo Pizza: 141 Park St, Vienna, Va 22180

Violations:

  • The following food item prepared from ambient air temperature ingredients is not being adequately cooled to 41F or less within 4 hours: 1) Canned tomato sauce portioned into 2.5 gallon containers observed in the walk-in refrigerator at 44-46F from the day before.
  • Observed an unapproved splitter on the mop sink faucet.
  • A food employee was observed cleaning their hands in the 3-vat sink.
  • Observed clean equipment found stacked while wet after cleaning and chemical sanitization on the shelves at the dish washing area
  • Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
  • Observed boxes of single service take home containers stored directly on the floor in the dish washing area of the kitchen.
  • The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: 1) shelving unit located above the 2door prep top refrigerator 2) small fan hanging above the 2door prep top.
  • The following food items that are not easily identified by appearance were observed without a label: 1) squeeze bottles of condiments, oil 2) shakers of dry seasonings 3) large bulk bin of flour.
  • Clean equipment observed stored with the food-contact surface facing upward while in storage.
  • Observed clean take home/catering containers found handled, displayed or dispensed with the food or lip-contact surface facing upward.
  • Observed that some areas of the kitchen walls are in need of cleaning.
  • Observed food and beverages directly stored on the floor in the walk-in refrigerator (Corrected). Observed beverages directly stored on the floor at the front counter area.
  • Observed one kitchen hand sink basin slow to drain.
  • Soda crate(s) found used for the following purpose(s): elevate pizza boxes off the floor in the hallway storage storage closet.
  • The nonfood-contact surface of the following equipment is not corrosion resistant, nonabsorbent, and/or smooth: 1) aluminum foil filing shelving units at the 2door prep top refrigerator 2) cardboard lining a dunnage rack where oil is stored in the kitchen.

Chipotle Mexican Grill: 213 Maple Ave, Vienna, Va 22180

Violations:

  • The following food item(s) were observed hot holding at improper temperatures using a calibrated food temperature measuring device: Observed chopped grilled chicken thighs (132F*) hot holding in warmer cabinet in front kitchen area (back cookline).
  • Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled 'Employee Health Policy.' Observed CFM unable to provide employee health policy in place. CFM only provide “red book” All reportable symptoms and foodborne illnesses (BIG 6) are not listed.

Sweet Leaf Cafe: 262 Maple Ave, Vienna, Va 22180

Violations:

  • The food establishment does not have written procedures for employees to follow when responding to vomiting or diarrheal events.
  • There is no Certified Food Manager present in the establishment. Manager holding the ServeSafe certified food manager certificate was not present during the inspection.
  • Observed the following food thawing using an improper method: Observed several cartons of Yo cream (premade yogurt mix, 110F) improperly thawing on counter premanufactured thawing instructions on carton for 30 mins. Carton states product should be thawed under refrigeration for 48- 72 hrs.).

Cava: 418 Maple Ave, Vienna, Va 22180

Violations:

  • Observed an unapproved splitter device on the mop sink faucet.
  • Employees are not using the dressing rooms or lockers provided. Observed employees personal belongings (e.g. back pack and purse) stored on shelving with equipment, food, and/or single service items.
  • Plumbing connections under the front service line hand sink equipment is leaking.
  • Observed some ceiling panels not maintained in good repair. Small gaps/holes were observed where electrical wiring, plumbing, etc were located.
  • Openings to the exterior of the building are present along the base of both the FRONT and REAR DOORS.

Anita's New Mexican Style Food: 521 Maple Ave, Vienna, Va 22180

Last inspection: July 2

Violations:

  • The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: 1) WHIPPED CREAM STORED IN THE BEVERAGE AIR 1DOOR UNDER COUNTER REFRIGERATOR AT THE BAR: 48F, 2) FOOD STORED IN THE 4DRAWER REFRIGERATOR UNDER THE MAIN COOK LINE GRILL: A) CHICKEN TAQUITOS: 45F, B) SMOKED SAUSAGE LINKS: 45F C) PRE-COOKED/SHREDDED BEEF: 51F.
  • Observed the pre-wash spray hose to extend below the flood rim level of the pre-scrape basin before the dish machine.
  • Observed the chemical sanitizing dish machine failing to deliver chlorine at the required 50 200ppm.
  • Observed no certified food manager on duty at the start of the inspection.
  • The following raw and/or undercooked foods are provided on the menu, menu board, table tent or brochure without proper disclosure: 1) FRIED EGGS, 2) 1 OR 2 EGGS, 3) STEAK (ANY KIND FOR FAJITAS, NY STRIP, ETC).
  • Observed some hand sinks being used for the storage of equipment, wet towel buckets, or for dumping.
  • Working containers of poisonous and toxic materials are not properly labeled with a common name.
  • The food-contact surfaces of the following equipment and/or utensils are not durable: 1) CARDBOARD LINING SHELVES IN THE MAIN COOK LINE AREA ABOVE THE 2DOOR PREP TOP REFRIGERATOR.
  • Observed gaps/holes in ceiling panels where soda lines, plumbing, electrical wiring or conduit lines are located.
  • Observed boxes of tortillas stored in direct contact with the floor in the stairwell leading to the basement storage area.
  • Observed equipment stacked while wet after cleaning and chemical sanitization.
  • The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41F: 1) BEVERAGE AIR 1DOOR UNDER COUNTER REFRIGERATOR AT THE BAR AREA OF THE KITCHEN OBSERVED AND MEASURED AT 45F, 2) 4DRAWER REFRIGERATOR LOCATED UNDER THE GRILL AT THE MAIN COOK LINE OBSERVED AND MEASURED AT 44F-46F.
  • Observed at the floor funnel located under the middle basin of the 3-vat sink not draining.
  • There was no temperature measuring device located in the following hot holding equipment: 1) CRES COR HOT HOLDING UNIT AT CARRY OUT, 2) HALO HEAT HOT HOLDING UNIT AT THE COOK LINE.
  • Dispensing and/or in-use utensils improperly stored between use as follows:1) IN-USE UTENSILS AT THE MAIN COOK LINE STEAM TABLE STORED IN WATER MEASURING 110F.
  • Observed single service lids for bar drinks found handled, displayed or dispensed with the food or lip-contact surface facing upward.
  • Clean ice bucket on top of the ice machine was observed stored with the food-contact surface facing upward.
  • Observed unwrapped coffee filters observed stored unprotected at the server station.
  • Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
  • Observed the server station and both restroom ceiling panels not ceiling is not designed to be smooth and easily cleanable.

Hissho Sushi at Navy Federal Credit Union: 203 Follin Ln, Vienna, Va 22180

Violations:

  • Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete.   • Raw animal food storage, preparation, holding or display conducted in a manner that may cause cross contamination of ready-to-eat food as follows: Observed raw shelled eggs stored above cups of RTE chopped watermelon inside of 2 dr preptop cooler.
  • Observed cooking equipment stored in hand sink located in the kitchen area, preventing access by employees for easy handwashing.
  • Observed that mops are improperly stored between use.
