• Excavator Bucket That Crushed Worker Leads to $170K Fine for Washington Contractor (insurancejournal.com) — A Woodland construction site has been cited after a 3,000‑pound excavator bucket fell and twice crushed a worker, leaving him hospitalized for a month. State labor officials say multiple excavators at the Woodland site and elsewhere lacked required safety latches, leading to $170,000 in fines and the contractor being placed on a Severe Violator list, which the company plans to appeal.
