Elmbrook Updates Student Trip Form Over Coronavirus Concerns
"We ask that all families use this updated form if their student will be missing school to travel," officials said this week.

BROOKFIELD, WI — As school districts across Wisconsin prepare in the event cases of the new coronavirus, also known as COVID-19, appear, the Elmbrook School District is asking parents of students to fill out a new form that includes where they are going and for how long.
On March 6, 2020, the School District of Elmbrook updated the Family Trip Request Form found on page 3 of practice statement 5113, to include the location of travel and dates.
"This will increase access to vital information in the event that the Health Department needs us to gather the information of any students who have traveled to areas that are identified by the CDC related to coronavirus (COVID-19)," school district officials said. "We ask that all families use this updated form if their student will be missing school to travel. Please contact your school health room with any questions."
