The City of Oak Creek Resident Priority Survey response period is open! If you received a survey in the mail, you can complete and return the paper form, or fill out the survey online with your survey ID number.

If you are a resident who did not receive a survey in the mail as part of our random sample, we'd still like to hear from you! Please use the link below and enter "Oak Creek" in the survey ID field to proceed with the survey. 

Your feedback will provide City leaders with the necessary information to ensure that tax dollars are being spent on the services you value.
