Homeless Action Partnership Rebranded In Santa Cruz
The county's local continuum of care has a new name to better align with its mission.
SANTA CRUZ, CA — Santa Cruz County’s federally-designated continuum of care (CoC) has been rebranded.
The former Homeless Action Partnership is now the Housing For Health Partnership (H4HP), the county said.
Changing the name was an effort to better align with the Housing for Health Division which works with the county’s human services agency and serves as the head CoC partnership agency.

The governance of H4HP has also been restructured, giving local jurisdiction officials and people with lived experience more direct involvement in policy decisions, the county said.
The new H4HP structure has a policy board consisting of 15 community leaders who represent constituencies with significant experience or resources to address the issue of homelessness, according to the county.
