Durham and Middlefield Residents Asked to Take Substance Abuse Survey
The Durham Middlefield Local Wellness Council was recently awarded a $500,000 Drug Free Communities grant. This survey will help the organization in its efforts to administer the grant and is completely anonymous.

Below is the link to the Community Readiness Survey that is required by the Drug Free Communities grant we received this year.

This survey asks about your own attitudes and perceptions of problems in the Communities of Durham and Middlefield regarding substance abuse as well as what you perceive to be the predominant attitudes in the communities. The results of our readiness survey will help us plan the future efforts of the Durham Middlefield Wellness Council by helping us benchmark current community perspectives regarding prevention.
Your responses to this survey are completely anonymous. You will not be asked your name or other identifying information. Your electronic address (IP address) will be stripped from your responses by the vendor who provides the survey software (SurveyMonkey.com). Results from this survey will only be released in aggregate form in which no individual's response can be identified.
