Sharon M. Herman, 43, of River Road, is charged with DUI and failure to drive right.
Police said Herman was arrested by warrant after a May 4 accident during which she crossed into the oncoming traffic lane.
Her bond was set at $5,000 for an appearance in Derby Superior Court.
